You must be specifically trained by the Stanford STARS course: DOT: Shipping Biological Goods or Dry Ice : EHS-PROG-2700 within the last two years to ship on Dry Ice. Does not authorize you to ship ANY OTHER hazardous materials.

Does the proposed shipment contain Dry Ice? (Do cryo training and fill out this shipping.pdf)

Does the proposed shipment contain Ethanol/Formaldehyde or any other hazardous substance?

FAILURE TO BE TRAINED AND PREPARE A HAZ SHIPMENT PROPERLY CAN RESULT IN:
Lost shipment, heavy fines, prison time. GET HELP WITH THESE!

ALL FOREIGN SHIPMENTS NEED MORE TIME. FOR HAZ MAT, MUCH. MUCH MORE TIME.

Gather the following information :

Name, short address w/ street address (minimum # of lines), phone # of recipient.

Weight, Dimensions of box, Contents.

Domestic or Foreign?

How soon does it need to get there? Overnight (Air) or ground okay?

Preferred method? USPS, UPS, FedEx, DHL, other?

Is the box strong enough? No styrofoam only boxes (must be covered in cardboard).
IF haz mat other than Dry Ice, Chris will have to use a special box/container and fully prepare the shipment.

If FedEx, need PTA number from Judy. Otherwise lab/grant/etc to charge to.

Non-haz materials can go straight to Doreen for normal shipping methods.

Dry-Ice shipments must be placed with Doreen ONLY by a trained person (within two years of training).
